ACTION ITEM: Consideration of a recommendation to the Village Board on the request of MENOMONEE FALLS JR PHOENIX BASEBALL INC to obtain a Conditional Use Permit allowing an indoor recreation facility located at N93W14605 Whittaker Way, Tax Key Number 5.005.003, I-1 Light Industrial Zoning District.

a. Consideration of a Certified Survey Map on the request of ADAM AND ELIZABETH KELLER to divide lands located at the southwest corner of Mill Road and Lannon Road. The subject parcel is located in the Northeast ¼ of Section 30 in the A-2 Agricultural/Urban Fringe Zoning District, and identified by Tax Key Number 117.999.

b. Consideration of preliminary and final approval of a land division, and a recommendation to the Village Board of Trustees regarding a Certified Survey Map redividing lands owned by JES GOOD FALLS, LLC. The Certified Survey Map is located on the south side of Good Hope Road between Highland Drive and 124th Street, on parcels identified by the following Tax Key Numbers: 93.975.004, 93.997, 93.975.003, 93.994, 93.993, 93.998, 93.999.001, 93.999, 93.987.003, 93.987.002 and 93.987.001

c. Conceptual review of a LASER CENTER FACILITY to be located on the Southeast corner of Main Street and Old Orchard Road, in the Westbrook Corporate Center, on a portion of the lands identified by Tax Key Number 45.991.
